Of all the numbers associated with heart health, cholesterol is probably the one people have heard of most. And yet it is also one of the most frequently misunderstood. I speak to customers regularly who have had a cholesterol reading and are not sure what to make of it – whether it is high, how worried they should be, or what it means in practice for their health. The confusion is understandable, because a cholesterol number on its own tells you less than most people realise.

Cardiovascular risk is not determined by any single measurement. It emerges from the combination of multiple factors – cholesterol, blood pressure, body composition, age, family history, smoking status, and more – working together over time. A total cholesterol reading of 6.0 mmol/L means something very different for a 45-year-old with well-controlled blood pressure and a healthy body composition than it does for a 55-year-old with hypertension and a high visceral fat score. What a home cholesterol test actually measures

Total cholesterol is a useful screening figure, but if your result is elevated, a full fasting lipid panel is the logical next step. Why total cholesterol alone is not the whole story

The relationship between cholesterol and cardiovascular risk is more nuanced than a single number suggests, and this is where a lot of the confusion I encounter with customers originates.

HDL cholesterol is genuinely protective. It acts as a scavenger, collecting excess cholesterol from the arteries and transporting it back to the liver for processing and removal. A person with a total cholesterol of 5.5 mmol/L but a high HDL component may have a lower cardiovascular risk than someone with a total cholesterol of 4.8 mmol/L and a very low HDL level. The ratio of total cholesterol to HDL is one of the figures clinicians find most useful, and it cannot be derived from a home total cholesterol test alone.

LDL cholesterol, and in particular a subtype called small dense LDL, is the primary driver of arterial plaque formation. Elevated LDL causes cholesterol to deposit in artery walls, triggering inflammation, narrowing the vessel, and progressively restricting blood flow. This process – atherosclerosis – underlies most heart attacks and strokes. Knowing your total cholesterol is useful; knowing your LDL is more useful still.

Triglycerides – fats carried in the blood that reflect dietary carbohydrate and sugar intake, as well as alcohol consumption – are a third independent risk factor that a total cholesterol test does not capture. Elevated triglycerides alongside elevated LDL represent a more dangerous combination than either alone.

None of this means home cholesterol testing is without value. It means understanding what it is – a screening tool that identifies who needs further investigation – rather than treating it as a complete cardiovascular risk assessment in itself.

Cholesterol and blood pressure – a compounding relationship

Of all the ways that cardiovascular risk factors interact, the relationship between elevated cholesterol and elevated blood pressure is one of the most clinically significant, and one of the least well understood by people managing both conditions.

High LDL cholesterol contributes to the build-up of plaques in artery walls. As plaques accumulate, arteries become stiffer and narrower. Stiffer arteries are harder to push blood through, which forces the heart to work harder and drives blood pressure higher.
